We will need to receive a referral from your physician or authorization from your IPA or insurance plan prior to scheduling you to attend our classes or other educational offerings. Please ask your referring physician to fax completed orders to us at 714.966.3338. Physicians may request a referral form by calling 714.966.8118.
Payment for Services
The Diabetes Center at Fountain Valley Regional Hospital accepts most major insurances. For information regarding your covered services and benefits, we suggest you contact your insurance carrier or IPA. Any co-pay or deductible that your insurance plan requires you to pay is due at the time of services.
