Information and education for diabetes management

You can live your life with diabetes under control, and the Diabetes Center at Fountain Valley Regional Hospital and Medical Center is here to help. The program helps adults and children learn how to manage their diabetes using several resources:

  • A Diabetes Self-Management Education Program accredited by the American Diabetes Association
  • Type 1 and Type 2 diabetes and gestational diabetes programs
  • Monthly support groups
  • Individual diabetes counseling and group classes available in English, Spanish and Vietnamese
  • An Outpatient Diabetes Self-Management Class, a six-week program on understanding medications, insulin pump therapy, nutrition and carbohydrate counts, exercise, self-management skills and more
  • Specialized care for patients with diabetic wounds through an Advanced Wound Center
  • An Advanced Inpatient Diabetes Care Program certified by The Joint Commission

Learn more about diabetes and take control of your health. The Diabetes Management Program at Fountain Valley focuses on self-management skills and personal health engagement. Program participants benefit from customized guidance and support.

How to Enroll

Please provide a referral from your physician or authorization from your independent practice association (IPA) or insurance plan if you want to attend our classes or other educational offerings. This is required before scheduling. Please ask your referring physician to fax completed orders to us at (714) 966-3338. Physicians may request a referral form by calling (714) 966-8118.

Payment for services

The Diabetes Center at Fountain Valley Regional Hospital and Medical Center accepts payments from most major insurance plans. For information regarding your covered services and benefits, we suggest you contact your insurance carrier or IPA. Any copay or deductible that your insurance plan requires you to pay is due at the time of services.
